How to Verify Email & View/Sign Paperwork:
You will need to verify your email before our system will allow you to fill out paperwork. If you have not verified your email you will not be able to see the paperwork, instead, the system will say, “Please Verify Email.”

Card on File Policy & Adding Card To Patient Profile:
As part of our updated office policy, we are requiring patients to have a credit card on file to establish care. Since we are seeing some patients remotely this helps us to collect things such as copay, coinsurance, and up-front payments that are due at the time of the appointment. It is our policy to not collect card information through intake paperwork so you will need to add your card to the billing section of the patient portal. You can also call our office and we can add the information to your profile over the phone.

Please contact our office once your paperwork is complete to schedule your appointment.
All new patient paperwork must be completed before we can schedule your appointment.
If your paperwork has not been started or is only partially complete we will not be able to see you.
