Patient Portal & Paperwork Help
How to Verify Email & View/Sign Paperwork:
You will need to verify your email when creating an account before our system will allow you to fill out paperwork. You will receive an email notification that paperwork has been added to your account and is ready for you to sign. The paperwork will be located under “Clinical Forms” and is separate from the additional demographic information you may have filled out when creating your account.
Card on File Policy & Adding Card To Patient Profile:
We require patients to have a credit card on file to establish care. Since we are seeing some patients remotely this helps us to collect things such as copay, coinsurance, and up-front payments that are due at the time of the appointment. You will be prompted to add your card to the billing section of the patient portal. You can also call our office and we can add the information to your profile over the phone.
Please contact our office once your paperwork is complete to schedule your appointment.
All new patient paperwork must be completed before we can schedule your appointment.
If your paperwork has not been started or is only partially complete we will not be able to see you.
