First Visit

Please call the Coeur Group to schedule an appointment. Because the Coeur group strives to be a virtually paperless practice, you will be set up over the phone with an individualized patient portal account. After this is completed, you will be sent, via the secure portal, practice documents to print and fill out to bring to your first visit. Additionally you will be asked to complete some assessments prior to your appointment.

Please bring your insurance information, and your insurance card with you.

Allow for at least 2 hours for your first visit, including the check in time.  The initial visit will cover a lot of information including past health history and how you feel today. Our providers want to make sure there is ample time for thorough evaluation, and identification of individual’s unique needs and their desired treatment goals. Length of follow up visits will be determined by your individual needs and goals. Your time is important to us, and we strive to give you prompt comprehensive services.
